Description & Services
Description Provides clinical and social services to the residents of the City of Vaughan * youth, seniors and people with mental health and addiction issues * services provided in different languages * serving those facing barriers accessing health care as a result of socio-economic status, geographic isolation, cultural or linguistic barriers * support groups * after-school teen and youth groups * health education * collaborations with community-based health and social service organizations  
Diabetes Education Program Supports for people living with prediabetes or type 2 diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * certified diabetes educators: Nurse and dietitian on staff * clients are also able to connect with diabetes educator for one-on-one counseling using their laptop or cellular device via OTN solutions * chiropody services available with referral from your physician or diabetes educator * insulin starts * glucose screening clinics 
Pulmonary Rehabilitation Program and COPD Clinic - supports people at risk or living with Chronic Obstructive Pulmonary Disease (COPD) * clients will have access to a registered Respiratory therapist, Registered kinesiologist and registered dietitian * services include breathing test, one-on-one counseling (including self management education, healthy eating, physical activity), smoking cessation program, individual and group exercise, 12 week pulmonary rehabilitation program, COPD support group
